How it works
From first call to care at home.
- 1
Reach out
Fill out our short request form or email us. We'll get in touch the way you prefer, learn about your loved one's needs, and explain what personal care at home could look like.
- 2
Get assessed
Personal care through Medicaid starts with a state assessment of needs. We'll explain what to expect and help you understand the process so nothing feels confusing.
- 3
Choose Keystone
When care is authorized, you choose the agency you want. Tell your case manager you'd like Keystone, and we'll take it from there.
- 4
Get set up
We set up a care plan, match a caregiver, and cover all required caregiver training. If a family member may be able to serve as the paid caregiver, we guide them through each step.
- 5
Care begins
Care starts on the schedule that's been authorized. We stay in touch, check in on how things are going, and pay caregivers on time.
